Do people know the difference between a dialect and an accent?

And do people in Spanish speaking countries (former colonies) and English speaking countries that were former colonies of England realize that they speak a dialect and not an accent? Spanish people can understand people in Latin America, but they don't really consider that to be high Spanish, because there are so many words and grammatical structures that are different. The same is true for British English and American English, though less than the Spanish example.
